(Why we collect your personal data and what we do with it) \nWhen you supply your personal details to the Clinic they are stored and processed for 4 reasons (the bits in bold are the relevant terms used in the Data Protection Act 2018, which includes the General Data Protection Regulation \u2013 i.e the Law):<\/p>\n

We need to collect personal information about your health in order to provide you with the best possible treatment. Your request for treatment and our agreement to provide this constitutes a contract<\/strong>.\u200b You can, of course, refuse to provide the information but if you do, we would not be able to provide you with the treatment.<\/li>\n
We have a \u2018\u200bLegitimate Interest<\/strong>\u2019\u200b in collecting the information because without it we could not do our job safely and effectively.<\/li>\n
We also think it is important for us to be able to contact you in order to confirm your appointment or update you on matters related to your medical care. Again this constitutes \u2018\u200bLegitimate Interest<\/strong>\u2019\u200b but this time it is your legitimate interest.<\/li>\n
Provided we have your \u200bconsent<\/strong>,\u200b we may occasionally send you general health information in the form of articles, advice or newsletters. You may withdraw this consent at any time \u2013 just let us know by any convenient method.<\/li>\n<\/ol>\n
We have a \u200blegal obligation<\/strong>\u200b to retain your records for 8 years after your most recent appointment (or aged 25 whichever is longer) but after this period you can ask us to delete your records if you wish. Otherwise we will retain your records indefinitely in order to provide you with the best possible care should you need to see us at some future date.<\/p>\n
Your records are stored on paper and in locked filing cabinets.<\/p>\n
We will never share your data with anyone who does not need access without your written consent. Only the following people will have routine access to your data:<\/p>\n

Your practitioner(s) so that they can provide you with treatment<\/li>\n
Our reception or administrative staff so they can organize the practitioners\u2019 diaries and coordinate appointments and reminders<\/li>\n<\/ul>\n
As part of our obligations as primary healthcare practitioners, there may be times we need to share your medical records with other healthcare practitioners e.g. GPs, Consultants, surgeons, or medical insurance companies. Where this is the case, we will always inform you first unless we are under a legal obligation to comply.<\/p>\n
From time to time too, we may employ consultants to perform tasks which may give them access to your personal data (but not your medical notes). We will ensure that they are fully aware this information must be treated as confidential and that they sign a non-disclosure agreement.<\/p>\n
